Welcome - Getting Started

The following screens will guide you through entering all the information needed to file a maintenance report for FOG interceptor maintenance. If at any time you make a mistake at one of the steps, don't worry, you will be able to correct it afterwards.

Below are a list of each of the steps we will guide you through. Once all the steps are completed, you will be presented the information for review and submission. Once you have submitted it you can choose to have a copy of the information emailed to you.

  1. Facility Information - We will ask you to choose the facility where the maintenance was performed.
  2. Interceptor Information - We will ask you about the details of the interceptor or trap.
  3. Pumping Information - We will ask you about the pumping/hauling company that did the maintenance.
  4. Disposal Information - We will ask you about the disposal facility, measurements, and when the maintenance was performed.

Review and Submit - Finished

Almost done. Now just review that the information is correct and submit the report. Once you submit the report you will be asked if you want a copy of the report emailed to you.
